Temper tantrums

I am a parent but I am asking this question as a caregiver. We have a child who is very intelligent and first impression and some of the time he is the sweetest boy ever. Recently, he has had violent outbursts that I think is brought on by having his routine changed. He is very controlling and if he does not get what he wants he trashes the room, hits the teacher, when I am brought in (Director) he attacks me by hitting, kicking, and biting. He has also threatens violence and then laughs. Sometimes if I offer him something to eat he will stop but he does seem to want to eat about every two hours. The family has someone who has observed him but he doesn't act that way around her. I really want to help him but I am very frustrated that no one seems to know how to help him. Have any ideas?
