Taking off Diaper

My wife and I are at our wits end and looking for help.
Our little 2 1/2 year old has started taking off her diaper during naps or when she wakes up in the middle of the night. Needless to say, this creates some horrific results.

Please tell us we aren't the only one out there with this problem.

Short of wrapping our child in duct tape, we don't know what to do.

Let us know if anyone out there has had this problem and what you did to solve it.
